FCA requires that students be in dress code each day. The dress code can be found in the Parent-Student Handbook. For the purchasing of uniforms, please fill out the form below. The Land's End website is linked below for images and further information about the uniforms.

Uniform Order Form

  • Please fill out this form for your child, as the school will be ordering the uniforms. Please submit one form per student.

    Each child will receive 5 regular tops, 5 regular bottoms, and one P.E. uniform which consists of a t-shirt and either mesh shorts or sweatpants.

    You will receive an email restating what was ordered. The email may come to your spam folder, so please look there for an email from mailer@factswebsites.com.

    Faith Christian Academy is not responsible for incorrect sizing. All reorders will require a $5.00 PER ITEM restocking fee from the parent/guardian.
